dc.description.abstractBackground: Ballarat Health Services (BHS), is a regional health service providing acute, sub-acute, residential aged care and community based services. This poster illuminates the experiences of family members in relation to the end of life care their loved ones received at BHS. This research is part of a larger project that will inform the development of an end of life framework to be implemented by the health service Methods: A qualitative interpretative design was adopted and data was collected via semi-structured open-ended individual interviews. Thirteen participants shared their very private thoughts and memories of the death of their loved one and five key themes emerged following analysis of data Findings: The first theme revolved around care received from the health care team during their loved ones’ final admission. The second focused on planning before and after death and awareness of their loved one’s wishes. The third theme interrogates communication between family members, the person who was dying, and the health care team. This included discussions about death and dying. The fourth theme considers awareness of death and how family members experienced this. The final theme relates to follow-up and bereavement support from the health service. Conclusions: The outcome of these interviews will inform the EoL framework which is being developed to integrate the elements of: access to Advance Care Planning, a tool to help health professionals identify patients who are dying, medical goals of care documentation, a Care of the Dying Management Plan and a range of other supporting tools and documents.en
